Family Medicine Physician - Health Equity for Adolescents in the Legal System (HEAL) - Part Time

Work locations

Crossroads Juvenile Center and/or Horizon Juvenile Center.

Schedule

21 hours per week - Day, Overnight, and Weekend.

Responsibilities

• Provide medical care to ACS adolescents keeping safety and security measures paramount.
• Take relevant patient history, identify patient needs, and make diagnoses.
• Develop and implement treatment plans using diagnostic data.
• Perform complete well-care examinations, including physical examinations.
• Perform complete gynecological histories and physical examinations.
• Write prescriptions and inform patients about medications prescribed; garner parent/guardian consents when needed.
• Complete medical forms and documentation related to patient care.
• Make and track all referrals to other agencies and providers, with follow-up care provided.
• Aid with phlebotomy, as needed, to ensure smooth patient flow.
• Administer immunizations, as needed.
• Distribute, refill, discard, and discontinue medication for patients.
• Complete Court Orders and all other ACS mandated paperwork.
• Evaluate and treat ACS employees (ONLY on an emergent basis).
• Provide health education on topics including, but not limited to, preventive medicine and counseling on chronic diseases, as well as parenting, prenatal, nutrition, family-planning, medication usage, and stress management.
• Participate in patient outreach.
• Participate in case conferences in which patient care is discussed.
• Auxiliary in the development and implementation of medical protocols and medical forms.
• Initiate and participate in research related to health care or allied fields, in collaboration with the Medical Director.

Qualifications

The ideal candidate will have a current NY State Medical License, DEA license, and must be Board Certified in Family Medicine. Experience working with adolescents in the juvenile justice system is preferred.
