100% outpatient; Monday - Friday opportunity with flexible scheduling
Patient Center Medical Home Model
Practice supported by two experienced APPs
Call is 1:5 telephone only, and supported by nurse advisory line
Qualifies for RIMSAP/IDPH loan obligation
Opportunity to teach Family Medicine residents and medical students
Opportunity for research and/or academic affiliation with the Carle Illinois College of Medicine

Responsibilities

• Provide high-quality primary care services to patients in a Patient Centered Medical Home model
• Collaborate with two experienced APPs to support clinical operations
• Manage a caseload that includes preventive care, chronic disease management, and acute care
• Conduct routine physical exams, order and interpret diagnostic tests, and prescribe medications as appropriate
• Teach Family Medicine residents and medical students
• Participate in research and/or academic activities

Requirements

• Board Certified/Board Eligible in Family Medicine or Adult Medicine
• Current Illinois Medical License
• Experience in a Patient Centered Medical Home model preferred
• Strong interpersonal and communication skills

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Completed residency in Family Medicine or Adult Medicine
