Family Medicine Nurse Practitioner/Physician Assistant

About the role

Seeking a Nurse Practitioner or Physician Assistant to provide comprehensive primary care in a collaborative, team-based environment. The role involves delivering full-spectrum family medicine services, managing acute and chronic conditions, ordering and interpreting diagnostic tests, prescribing medications, and offering preventive care and patient education.

Responsibilities

• Conduct patient evaluations and coordinate referrals.
• Collaborate with healthcare team members to ensure continuity of care.
• Maintain accurate clinical documentation per organizational standards.

Requirements

• Active license and registration in New York State as a PA or Nurse Practitioner.
• For PAs: Bachelor's degree in physician assistant studies, certification from NCCPA, and at least one year of ambulatory care experience.
• For NPs: Completion of an approved nurse practitioner program, certification from AANPCB or ANCC, and one year of ambulatory care experience preferred.
• Current CPR certification required.
