About the role
Seeking an Advance Practice Provider to provide comprehensive primary care services, including health promotion, disease prevention, and management of acute and chronic illnesses for patients of all ages.
Responsibilities
- Cover provider patient panels during leaves of absence
- Perform acute telemedicine visits from a home-based practice
- Work in office and examination room settings
- Travel to other practice locations as needed
- Adhere to infection control policies and PPE use in all work environments
Requirements
- Current New York State RN and Family Nurse Practitioner license, or
- Current license to practice as a Physician Assistant in New York

This role is within a nationally recognized, patient-centered medical home practice, part of a large multi-specialty group with extensive referral networks.

Pay
Salary ranges from $131,900 to $155,200 annually.
