About the role
Coordinates the multidisciplinary care delivered to their panel of patients, to optimize patient care. Reviews patient medical records to diagnose, treat, and provide care management and education to patients and families. Makes appropriate patient referrals. Champions a culture of empathy, dignity, and respect for aging populations. Monitors national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care. Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior. Maintains a collaborative environment with other referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care. Supports academic, educational and research endeavors. Assesses and manages chronic and acute conditions in patients with multiple comorbidities. Partners and communicates with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group. This would include health network staff, vendors, outside partners, patients, stakeholders, and local community leaders. Serves as a resource to the community.

Requirements
- Current Medical Doctor (M.D) or Doctor of Osteopathy (D.O.) degree from an accredited school of medicine.
- Residency and/or Fellowship training in Family Medicine (Geriatrics).
- Current, unrestricted medical license to practice in the State of Texas.
- Board Certification or Board Eligible.
- Licensure/Certification/Specialized Training: Current, unrestricted medical license to practice in the State of Texas. Board Certification or Board Eligible.
Qualifications
- Required Education And Experience: Current Medical Doctor (M.D) or Doctor of Osteopathy (D.O.) degree from an accredited school of medicine. Residency and/or Fellowship training in Family Medicine (Geriatrics).
- Required Licensure/Certification/Specialized Training: Current, unrestricted medical license to practice in the State of Texas. Board Certification or Board Eligible.
- Unrestricted work authorization to work in the United States.
Skills
- Bilingual or multilingual proficiency is a plus.
- Prior experience in post-acute or long-term care settings strongly preferred.
- Proficient with electronic medical records and knowledgeable of CMS/SNF regulatory requirements.
