Essential Job Functions

  • Works with the full family in a collaborative and supportive manner to help, motivate, and inspire individual and family goal setting while allowing the family to drive the work.
  • Helps the family to identify strengths and then work with the families to utilize these strengths and build others.
  • Uses trauma informed practices.
  • Provides services according to values and principles for family centered coaching.
  • Completes the intakes and monitors family goals and activities.
  • Completes senior assessments and monitors goals, activities, and referrals.
  • Provides follow up to ensure family goals are progressing or achieved.
  • Focuses on addressing foster economic independence, increasing earning power, supporting academic achievement, enhancing stability and quality of life, coordinating events and activities, addressing housing related issues, assisting families with meeting site-based requirements and CHA work requirements, providing support services, recording family progress, handling emergency and crisis situations, working within contract, grant, and program requirements, utilizing a team approach, and working with other program staff.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Works effectively with diverse staff and service populations.

Knowledge, Skills and Abilities

  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Mobility to work with community-based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Highly organized and knowledge of Excel to track data.

Qualifications

  • High School Diploma or equivalent, required.
  • 2+ years' experience in case management, which includes the coordination of services and housing needs, required.
  • Bachelor's degree in social work or related field from an accredited college or university, preferred.
  • Bilingual preferred.

Additional Requirements

  • Satisfactorily pass all relevant screenings and criminal background checks.
  • Driving for work required with a personally owned vehicle.
  • Travel between sites required.

Physical Demands

  • Sit and stand for sustained periods of time.
  • Frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls.
  • Occasionally stand, bend, stretch, stoop, kneel and crouch.
  • Close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.

Pay

The average starting salary for this position will fall in the range of $44,000 and $49,500 annually. Candidates will be placed in this range based on skill and experience level.
