Every Other Weekend Hospice Nurse - LVN

This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Responsibilities

• Answer calls from patients, families, and customers.
• Complete patient visits in private residences, assisted living communities, and skilled nursing facilities.
• Demonstrate passion for hospice care and act as an advocate for hospice services and Suncrest Hospice.

Requirements

• Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure (in good standing) in the state(s) of practice.
• Great clinical and communication skills.
• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1‑to‑1 basis while receiving strong support from the patient care team.
• Ability to work independently, managing day‑to‑day activities such as phone, email, written communication, meetings with various healthcare professionals, and understanding hospice rules and regulations.
• Strong time‑management skills to schedule visits, attend required meetings, and prioritize/triage urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a valid driver’s license, and car insurance.
