Every Other Weekend Hospice On-Call Registered Nurse, RN

Suncrest Hospice is dedicated to changing the expectation of hospice care by providing exceptional service and increasing staff-to-patient ratios, resulting in more clinical visits and lower clinician caseloads. We are a Community Health Accreditation Partner (CHAP) certified hospice.

About the role

This weekend RN position supports our on-call needs, working every other weekend (Saturday and Sunday) in 10-hour shifts. The role involves providing care and services to hospice patients in their residences, including private homes, assisted living facilities, and hospitals.

Responsibilities

• Provide care and services for a group of hospice patients, including triage, symptom management, and PRN needs.
• Collaborate daily with a multidisciplinary team, including nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1:1 level while receiving support from a patient care team.
• Manage independent day-to-day activities, including phone, email, and written communication, as well as meetings with healthcare professionals.
• Understand and adhere to hospice rules and regulations.
• Demonstrate strong time management skills to prioritize visits, documentation, meetings, and urgent needs.
• Learn and utilize an electronic medical records system.
• Drive across the service area using reliable transportation, with a valid driver’s license and auto insurance.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice, with at least one year of experience under license.
• Admissions and on-call experience preferred.
• Ability to work independently and as part of a team.
• Availability to work every other weekend (Saturdays and Sundays) with the opportunity to pick up shifts on Thursdays and Fridays.
