Every Other Weekend Hospice LVN

About the role
As a Weekend LVN/LPN, you will complete patient visits as needed. Visits occur in patient homes including private residences, assisted living communities, and skilled nursing facilities. This position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.
- Answer calls from patients, families, and customers
- Work closely with patients on a 1-1 level while receiving strong support from a patient care team
- Work independently with activities including phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations
- Manage time effectively to handle visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns
- Learn and use an electronic medical records system
- Drive with reliable transportation (Driver’s License and car insurance required)
Requirements
- Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure (in good standing) in the state(s) of practice
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Great clinical and communication skills
Pay
$40-45/hour
