About the role
Piedmont Health Senior Care is dedicated to enhancing the lives of seniors in the community through the Program of All-inclusive Care for the Elderly (PACE). We help seniors maintain their independence and live at home for as long as possible by offering comprehensive, personalized healthcare and related services tailored to each senior's unique needs and aspirations.
Responsibilities
- Evaluates potential participants to the program to determine participant needs and eligibility for enrollment.
- Completes initial assessments in the homes of potential participants, including evenings and weekends as needed.
- Affirms appropriate assessments using clinical observation, medical records, and verbal information, interaction with patient/family, and care team.
- Collaborates with State Medicaid (or designee) and Medicare applications with county Department of Social Service personnel.
- Participates in interdisciplinary treatment teams (IDT), management teams, and other related meetings and activities.
- Conducts outreach activities in conjunction with the Marketing and Enrollment Manager.
- Maintains statistical data required and presents Intake reports at weekly staff meetings.
- Travels to client's homes and/or other agencies in the community as needed.
- Oversees facilitation of the Care Plan and Enrollment Agreement in collaboration with the Nurse Care Manager and Team Director.
- Engages in community outreach events and presentations.
- Provides alternative community resources to applicants who are not accepted into the PACE program.
- Contacts identified key referral sources regularly to discuss possible program candidates.
- Maintains ongoing relationships with identified individuals and agencies to ensure a clear understanding of PACE as a vital community resource.
Requirements
- Education/Experience: 3-year degree from a recognized college or university, or equivalent combination of experience.
- Strong preference for knowledge of the community served by Piedmont.
- Demonstrated success in meeting sales benchmarks.
- One year of experience with the frail or elderly preferred.
- Fluency in Spanish.
- Licensure, Registry, or Certification Required: Valid NC Driver License and CPR/BLS certification.
- Immunizations: Medical clearance for communicable diseases and up-to-date immunizations.

Pay
$22.49/Hourly - $30.23/Hourly (commensurate with years of experience)
Schedule
Monday through Friday, 8:00am to 5:00pm
