About the role

We are seeking a social worker with geriatric experience to join our team of Elder Care Coordinators. The ideal candidate will have 3-5 years of experience as a social worker specializing in geriatrics, possess a strong sense of care and compassion, and live locally.

Responsibilities

  • Serves as advocate and experienced eldercare professional on the Legal Team.
  • Provides client care assessments to identify parameters of care and capacity.
  • Interfaces with the family and clients by making visits and communicating frequently.
  • Assists family to gather and sort important documents necessary for Medicaid and other planning purposes.
  • Counsels family on care issues and concerns.
  • Translates “legal-speak” and “long term care speak” to client/family.
  • Advises legal and administrative team on current, changing, and predicted issues for the family.
  • Represents the law firm to facilities and healthcare community by visiting regularly.
  • Is knowledgeable about care options in the community.
  • Makes regular visits to staff of professional organizations.
  • Participates in professional meetings.
  • Presents law firm services to professionals and potential clients.
  • Contributes information and articles to newsletter and website.
  • Manages aspects of databases for newsletter and mailings.
  • Helps create CEU programs and get those programs accredited with the appropriate boards.
  • Works closely with integrating best practices as appropriate.
  • Represents the firm on boards and committees as appropriate.
  • Participates in local and national organizations.
  • Supports the firm in development of firm presentations and materials to local organizations, as well as client presentations and in-service presentations.
