A residential support role providing care and companionship to individuals with disabilities, assisting in the development and implementation of personalized Support Plans, and helping them achieve personal goals and independence.
Responsibilities
- Direct Care & Support (85%)
- Provide daily support, care, and companionship aligned with individual Support Plans (ISPs) while maintaining dignity and respect.
- Assist with personal care, including bathing, dressing, bathroom assistance, and housekeeping.
- Prepare and serve meals according to individual diet plans.
- Administer medication and prescriptions under the direction of a nurse.
- Schedule and transport individuals to medical appointments, outings, or errands as requested.
- Support engaging activities to enhance daily living and help individuals achieve personal goals.
- Maintain accurate record-keeping, including general event reporting (GER), medical orders, state compliance requirements, abuse reporting, and internal procedures.
- Program Support (15%)
- Contribute insights into daily activities and individual interests during the development and updating of Support Plans.
- Purchase food and supplies as instructed.
- Maintain a clean, safe, and healthy residential environment, ensuring compliance with safety protocols.
- Attend staff meetings and participate in continuing education.
Requirements
- High School diploma or equivalent preferred (not required unless mandated by state regulations).
- No prior experience required.
- Program-specific licensure or certification may be required based on state regulations (e.g., Driver’s License with a clean Motor Vehicle Record).
