About the Role
The Transition & Intake Director at Life Unlimited ensures that adults with developmental disabilities receive comprehensive and personalized support services. This role supervises Service Navigators (SNs) and guides individuals through the agency's array of services, facilitating access to resources and fostering a supportive environment that empowers individuals to achieve their goals. The role also involves establishing and nurturing connections with local community partners to promote the agency's mission and raise community awareness.
Responsibilities
- Quality
- Establish and maintain the Intake process and corresponding documentation within the agency, including checklists and QA points to ensure all tasks are completed for any situation or program intake.
- Create a centralized electronic location for intake process and documentation (e.g., Therap, SharePoint, Teams) and ensure all necessary parties can access and view information as needed.
- Monitor utilization of intake process documentation by SNs.
- Monitor the current Intake Waitlist to ensure data is current. Analyze data in the Intake Waitlist monthly for service inquiry and trends and present data to Program VPs and COO.
- Act as the main liaison, in conjunction with SNs, to navigate individuals through the various services offered by the agency, ensuring seamless access to programs, therapies, and community resources.
- Develop and maintain procedures to ensure comprehensive assessment of individuals' needs, strengths, and preferences during the intake process. Ensure data compiled during intake assessments are visible to necessary team members and documented for future reference.
- Ensure individuals have established individualized service plans with achievable goals for any Life Unlimited services within 90 days of intake.
- Coordinate and advocate for necessary services, including medical care, educational support, vocational training, and social activities. Ensure connection to these services occurs in a timely manner.
- Serve as an advocate for individuals, ensuring their voices are heard in decision-making processes.
- Empower individuals to make informed choices about their lives during the intake and transition process, fostering independence and self-determination.
- Demonstrate a person-centered philosophy and language when discussing service and placement options within the agency.
- Growth
- Identify potential partnerships with local schools, medical facilities, nursing homes, and other community resources.
- Educate community partners on the agency mission and scope of services; integrate agency staff into transition processes with community partner agencies for individuals (e.g., IEP and transition meetings, discharge meetings).
- Lead the weekly agency intake meeting with all agency departments. Establish intake timeline expectations and ensure placements progress within expected timelines across all departments.
- Develop and maintain procedures to ensure daily monitoring of the Consumer Referral Database and quick correspondence with individuals and team members to promote placement activity.
- Provide supervision and support to Service Navigators between weekly intake meetings, exploring potential placements, anticipating program questions/concerns, and gathering all necessary information prior to weekly intake meetings.
- Track and trend all referrals and intake work within the agency. Report data and analysis to Program VPs and COO monthly.
- Team Builder
- Provide supervision and guidance to team members within the Service Navigation Department.
- Establish processes within the agency to facilitate smooth transitions into Life Unlimited services. Identify transition points within the intake process and establish roles and responsibilities.
- Oversee the intake process, ensuring efficiency and accuracy in assessing individuals' needs and directing them to appropriate services.
- Maintain and implement all necessary forms for the department. Work with QA and IT to ensure necessary data is published and viewable to enable assessment of QA and growth.
- Work closely with support departments (Housing, Finance, Training) to ensure all team members are informed of referrals or upcoming placements. Incorporate support department needs into intake checklists and procedures.
- Communication and Community Connection
- Conduct training sessions for individuals, families, and caregivers on available services, self-advocacy, and community resources.
- Establish and maintain strong, frequent communication with all individuals or team members inquiring about services at Life Unlimited. Follow contact until resolution of need.
- Along with the Service Navigators, connect individuals and team members to appropriate agency personnel. Follow up on connections until services begin and the individual is fully integrated into programs.
- Establish and maintain strong relationships with families, caregivers, and external service providers to create a comprehensive support network.
- Collaborate with interdisciplinary teams to ensure a holistic approach to individualized care.
Physical Expectations
- Extended periods of walking and standing with some periods of sitting, depending on the needs and activities of the individuals supported.
- May require light to heavy lifting and occasional stooping, kneeling, bending, and/or climbing stairs, depending on the needs of the individual supported.
- Occasional physical intervention if confronted by a person displaying combative or potentially dangerous behavior.
- Office/home environment with noises from appliances, office equipment, and facility/maintenance team.
- May be exposed to bloodborne pathogens and/or infectious diseases.
- Frequent meetings with individuals in their homes.

Qualifications
- Bachelor’s Degree in a Human Service Field preferred.
- Minimum of 5 years of management experience, with one (1) of those years working with individuals with developmental disabilities strongly preferred.
- Must obtain and maintain training and certification in required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, TOOLS, and Crisis Intervention.
- Background in the skills of developmental, implementation, and the writing of teaching strategies.
- Must have a reliable personal vehicle for transporting clients.
- Must obtain a Class E driver’s license during new hire orientation and carry minimum coverage applicable by law for auto insurance.
- Must receive seasonal flu vaccination.
Pay
Salary starting range: $65,000 DOE
