Director of Social Services (LSW CSW)

Qualifications & Responsibilities

• Must have LSW or CSW
• Previous long term care experience is preferred
• Prior experience managing social work staff is preferred
• Directs, establishes & plans the overall policies/goals for the Social Services Dept
• Maintain accurate, thorough, and timely documentation
• Participates in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Reviews Advanced Directives and provides education to residents and family members as needed
• Helps provide a smooth admission and discharge process for residents and family members
• Able to work independently or part of an interprofessional team
• Computer knowledge and experience using Point Click Care
• Other duties as assigned by Administrator

Schedule & Benefits

• Schedule: Full-time, day shift position, including Monthly Manager on Duty weekend rotation
