Schedule & Benefits

Responsibilities
- Directs, establishes, and plans the overall policies/goals for the Social Services Department
- Maintain accurate, thorough, and timely documentation
- Participates in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Review Advanced Directives and provide education to residents and family members as needed
- Help provide a smooth admission and discharge process for residents and family members
- Ability to work independently or as part of an interprofessional team
- Computer knowledge and experience using Point Click Care
- Other duties as assigned by Administrator
A wide degree of creativity and latitude is expected.
Requirements
- Must have LSW or CSW
- Previous long-term care experience is preferred
- Prior experience managing social work staff is preferred
