Social Services Director, LSW

About the Role

Villa Springfield is seeking a compassionate and dedicated Social Services Director to join our team. This full-time position plays a vital role in supporting the emotional, social, and psychosocial well-being of our residents and their families. Responsibilities

• Assess residents’ psychosocial needs and develop appropriate care plans
• Provide counseling and support to residents and families
• Coordinate discharge planning and community resource referrals
• Advocate for residents’ rights and well-being
• Collaborate with nursing staff, administration, and interdisciplinary teams
• Assist with care conferences and family communication
• Maintain accurate documentation and compliance with regulatory requirements

Requirements

• Licensed Social Worker (LSW) preferred but not required
• Candidates with relevant social services or healthcare experience will be considered
• Strong interpersonal, communication, and organizational skills
• Compassionate approach to resident care
• Ability to work effectively as part of an interdisciplinary team
