About the role

Provides hospice care services to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.

Responsibilities

• Visits patients in their home to provide comfort and support.
• Assesses patient needs and develops care plans.
• Collaborates with interdisciplinary team to ensure comprehensive care.
• Administers medications and treatments as prescribed.
• Maintains accurate records and reports.

Requirements

• Bachelor's degree in related field (nursing preferred).
• Current license to practice as a registered nurse.
• Experience in hospice care or similar healthcare setting.
• Strong communication and interpersonal skills.
