The Director of Care Management & Social Services is responsible for the operations of CHOC Children's Case Management and Social Services Departments. This role works with various systems and departments to ensure improvements in coordination of care and care delivered across the continuum. Administers all patient care services including oversight and development of processes related to complex patient and family psychosocial issues provided by the Case Management and Social Services Department. Serves as a resource to CHOC and the community regarding services provided by the departments.

Responsibilities
- Oversee operations of Case Management and Social Services Departments.
- Collaborate with various systems and departments to improve coordination of care across the continuum.
- Administer patient care services, including development of processes for complex psychosocial issues.
- Serve as a resource to CHOC and the community regarding departmental services.
- Interpret cost data and develop, prepare, or interpret graphics, statistics, and financial data.
- Solve complex clinical problems and develop strategies for a changing healthcare environment.
- Set goals, promote teamwork, and advance care through new ideas and technology.
Requirements
- Minimum five (5) years of clinical experience.
- At least three (3) years of management experience.
- Graduation from an accredited school of Registered Nursing.
- Bachelor’s degree in Healthcare and Master’s degree in healthcare or business.
- Experience with process improvement, analysis, and change implementation of business processes.
- Advanced communication and interpersonal skills with all levels of internal and external customers.
- Analytical ability to interpret cost data and develop financial graphics/statistics.
- Mental stability to cope with complex problems involving patients, staff, physicians, visitors, or external agencies.
- Independent judgment to solve complex clinical problems and balance patient care with institutional needs.
- Competency in Microsoft Office products (Word, Excel, MS Project).
Preferred Qualifications
- Experience in Case Management, Managed Care, or Community Clinic setting.
- Bilingual.
- Desire and ability to work with children and families in a multi-cultural environment.
- Certified Pediatric Nurse (CPN).

Licensure
- Basic Life Support (BLS) – required within 30 days of employment or transfer.

Work Environment

- Physical activities include: bending, carrying objects, climbing, keyboard use/repetitive motion, pinching/fine motor activities, pushing/pulling, reaching (forward/overhead), sitting, squatting/kneeling/crawling, standing, talking/hearing, twisting, walking, wrist position deviation.
- Sensory requirements: color discrimination, depth perception, far/near vision, hearing.
- Environmental exposure: airborne communicable diseases, bloodborne pathogens, chemicals, dust particulate matter, extreme noise levels, extreme temperatures, radiation, uneven surfaces or elevations.
Pay
Pay Range:
- Minimum: $154,918
- Midpoint: $205,234
- Maximum: $255,590
Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.
