Schedule

• Full time

• Monday - Friday 8:30am - 5pm

Location: 1035 RED BUD RD NE, CALHOUN, Georgia, 30701

Responsibilities

• Directs daily operations of the care management department, including discharge planning, care coordination, progression of care, and social services while ensuring compliance with CMS CoPs and federal/state regulations.
• Oversees recruitment, retention, staffing, and leadership development activities to ensure strong care management team performance.
• Collaborates with interdisciplinary hospital leaders, executives, post-acute care entities, and Utilization Management to improve patient outcomes, care transitions, and reduce readmissions.
• Identifies and leads operational, workflow, and technology improvement initiatives to enhance staff efficiency, patient outcomes, and departmental performance.
• Develops, implements, and ensures compliance with care management policies, standard operating procedures, operational strategies, and fiscal accountability measures.

Requirements

• Knowledge of change management principles, methodologies, and tools [Required]
• Master's degree in Social Work OR Bachelor's in Social Work & Master's in related field [Required]
• 5+ years direct care management experience [Required]
• 3+ years care management leadership experience [Required]
• 2+ years social work experience [Required]
• Additional licensure or certification requirements may apply depending on the specific unit or state.
