Director- Care Coordination and Population Health

Qualifications

• Master’s degree in nursing and valid Ohio Registered Nurse licensure
• 5 years progressive management experience in a healthcare related organization
• Recent experience in care management
• Experience leading new initiatives
• Knowledge of and experience with hospital information systems
• Knowledge of and experience with using financial systems to analyze financial performance
• Demonstrated initiative and evidence of continuing professional development
• Well-developed verbal and written communication
• Strong analytical skills
• Knowledge of and experience with the legal review of contracts
• Ability to interact effectively with various cultural, ethnic, racial, and socioeconomic backgrounds

Preferred

• 10 years progressive management experience in a healthcare related organization
• Hospital and physician practice managed care experience
• Knowledge of and experience with using financial systems to analyze financial performance

Physical Demands

May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job related documents. Ability to use computer.
