Primary Purpose
To provide direct care in a residential setting to participants with special needs to enable their highest quality of life in terms of their physical, emotional and mental health while adhering to CCARC policies and procedures.
Essential Functions
- Provides and receives effective, respectful, appropriate, and courteous verbal and written communication to and from specific teams related to the care of participants, including daily observations and reports of any physical or behavioral changes while adhering to all documentation and record keeping requirements.
- With family members, guardians, DDS and external healthcare providers including coordinating health and hygiene-related appointments.
- Provides essential physical and emotional care to participants including:
- Aiding with personal care, hygiene, and treatments including administration of medications, recording vital signs as needed, placing supply and medication orders.
- Providing education to participants in personal hygiene, grooming, and behavioral conduct while acting as a role model to enhance participants’ self-esteem.
- Cooking and providing nutritional support: Monitoring residents at mealtime according to food consistency guidelines for each individual.
- Coordinating and leading social and recreational activities as well as shopping for food and clothing, including the safe transportation of participants.

- Maintaining good daily attendance (days of absence, illness, tardiness).
- Attending and participating in mandatory in-services, staff meetings and conferences as required.
Education And/or Work Experience Requirements
- A High School Diploma / GED.
- A valid Driver's License, be insurable with Agency carrier.
- Individual Home Support employees are required to have a vehicle for use and maintain adequate insurance.
- Must complete the Agency’s Employee Training.
- Willingness to become trained in CPR, First Aid, PMT (Psychological/Physical Management Training) and DDS Medication Administration, if not already.
- Ability to follow oral and written instructions.
- Experience with Microsoft products, use of email and Therap preferred.
- Must be fully vaccinated against COVID-19.
