Direct Support Professional II (Homes)

About the role

This role operates in a Home and Community-Based Services (HCBS) Setting, providing medical, social, and supportive services for individuals with functional, cognitive, and other physical or mental health needs. Direct Support Professionals II are integral members of the care team, collaborating with other professionals to ensure the best possible care for those they serve.

Responsibilities

• Collaborate with local leadership to develop individualized goals and provide input on attainable objectives.
• Train Direct Support Professionals and model effective teaching strategies.
• Support staff orientation and training, assist with appointment coordination, and ensure team resources are current and readily available.
• Deliver individualized services by teaching skills, implementing behavioral support strategies, and motivating individuals to achieve their goals.
• Provide quality personal care, medication support, and maintain accurate, timely documentation of services, progress, and outcomes.
• Promote dignity, independence, and self-determination by advocating for individual choice, respecting rights, and ensuring services are delivered in a safe, empowering, and least-restrictive manner.

Requirements

• Minimum age requirement of 18 years of age.
• Driver’s License and reliable transportation to transport individuals to opportunities and activities.
• Ability to lift up to 35 pounds and wear hearing and eye protection as needed.
• Knowledge of the field and ability to communicate effectively.
• Confidence in balancing boldness and humility.
• High school diploma or equivalent.

Pay

The base pay is $17/hour. With education and experience, you could start out making more than that.
