Responsibilities

• Assist individuals with daily living activities and community involvement
• Encourage independence while providing compassionate support
• Administer medications after training and according to agency policies
• Document services provided and progress toward individual goals
• Monitor health needs and report changes when necessary
• Follow individualized care plans
• Safely transport individuals to appointments and community activities
• Complete required documentation, mileage logs, incident reports, and other records accurately and on time

Qualifications

• Valid driver's license
• Reliable personal vehicle available for client transportation
• Current automobile insurance
• High school diploma or GED
• Legal authorization to work in the United States
• Basic computer skills
• Ability to pass the Family Care Safety Registry screening and criminal background check
