What You'll Do (Essential Job Functions)

• Follow individual care plans and report any concerns to nursing or management.
• Administer medications and treatments with proper documentation, per policy.
• Support daily living needs, including:
• Personal care
• Meal preparation and dietary support
• Household tasks and errands
• Health appointments and community integration
• Build natural supports and social relationships.
• Promote health, safety, and a clean home environment.
• Remain calm and effective during challenging situations.
• Foster a positive, respectful team dynamic with coworkers, individuals, and their families.
• Accurately document individual progress and changes.
• Attend all required trainings, meetings, and ongoing professional development.

What You'll Need (Minimum Requirements)

• A high school diploma or GED.
• A valid driver's license and reliable transportation.
• The ability to lift 25-50 lbs. (with assistance or lifting devices).
• A physical ability to bend, squat, push/pull, and kneel as needed.
• A compassionate and patient mindset.
