About the role
Hybrid position combining in-clinic and remote work in Jacksonville, FL.
Responsibilities
- Provide in-person and virtual physical therapy services for diverse orthopedic and neurologic conditions.
- Develop personalized care plans and objectively measure patient outcomes using advanced software.
- Deliver telehealth evaluations, assessments, and treatments via digital platforms.
- Perform Remote Therapeutic Monitoring (RTM) and maintain effective communication with patients and providers.
- Support digital health program development, data analysis, and regional service growth.
- Collaborate with clinicians and the Digital Health team to optimize remote services.
Requirements
- Diploma from a CAPTE-approved Physical Therapy Program and active state licensure in Florida (or in process).
- Minimum of 2 years outpatient clinical experience; digital health experience preferred.
- Strong dedication to quality patient care and excellent communication skills.
- Ability to adapt to digital health technologies and deliver remote services effectively.
