Digital Health Specialist - Hybrid

About the role

Provide outpatient physical therapy services for diverse orthopedic and neurologic conditions in a hybrid setting, integrating digital health tools to enhance patient care.

Responsibilities

• Develop and implement personalized care plans, measuring patient outcomes with advanced software.
• Conduct evaluations, treatments, re-evaluations, and discharge documentation efficiently.
• Utilize digital platforms to deliver telehealth services, including assessments and treatment planning.
• Perform Telehealth and Remote Therapeutic Monitoring (RTM) services with interactive communication.
• Collaborate with clinical teams and support operations to deliver digital care, communicating treatment revisions as needed.
• Serve as a liaison between providers and the Digital Health Director to optimize remote services.
• Maintain regular reporting, data analysis, and participate in program development and marketing efforts.

Requirements

• Diploma from a CAPTE-approved Physical Therapy Program.
• Florida physical therapy license (or in process).
• Minimum 2 years of outpatient clinical experience; digital health experience preferred.
• Commitment to delivering high-quality patient care.
• Strong communication, collaboration, and digital platform skills.
