Digital Health Specialist - Hybrid

Responsibilities

• Provide outpatient physical therapy services for patients with orthopedic, neurologic, and other conditions.
• Develop and implement personalized care plans, utilizing digital health tools for virtual assessments and treatments.
• Deliver telehealth visits, including evaluations, assessments, and remote therapeutic monitoring.
• Objectively measure and document patient outcomes using advanced software.
• Collaborate with in-clinic providers and serve as a liaison to optimize remote care services.
• Support program development, marketing, and data analysis of digital health metrics.

Qualifications & Skills

• Diploma from a CAPTE-accredited Physical Therapy program and active Florida license (or in process).
• Minimum of 2 years outpatient clinical experience.
• Experience with digital health services preferred.
• Strong commitment to delivering high-quality patient care.
