About the role
SpecialCare Hospital Management is looking for a personable, results-oriented, and confident individual to serve as a Detox Intake Coordinator. This role involves screening and assessing patients to ensure they meet service criteria, as well as developing tailored discharge plans for those who have completed treatment.
Responsibilities
- Screen patients via phone and walk-in inquiries, initiate insurance verification, and schedule assessments and intakes.
- Complete all paperwork clearly, concisely, and in line with policy and procedure.
- Communicate effectively with hospital staff (physicians, nursing, etc.) regarding patient assessment, continued stay, and discharge planning.
- Maintain HIPAA compliance.
- Conduct in-facility rounds and follow-up calls on patients daily according to policy and procedure.
- Formulate discharge and aftercare plans with patient involvement.
- Report daily census to the corporate office.
- Perform community outreach telephone calls daily to various agencies.
- Complete biopsychosocial assessments.
- Perform other related duties as determined by the supervisor.
Requirements
- Bachelor’s degree in Human Services with at least 2 years of social service and/or clinical service work.
- Knowledge of addiction and/or dependence.
- Substance abuse counselor and/or case manager experience, or previous experience in substance abuse withdrawal, MAT Treatment, or detox is a plus.
- Ability to work as a team player, accept and promote goals with minimal supervision, and assist and support others.
- Strong problem-solving skills and the ability to implement solutions.
- Ability to handle stressful situations and interact effectively with others.
- Past experience in a hospital setting and hospital management is preferred.
- Satisfactory drug screen and criminal background check.
