Responsibilities
- Ensure company standards as a quality care provider through the delivery of excellent care and exceptional customer service.
- Conduct in-depth phone assessments to identify each client's unique care needs—from health conditions to emotional concerns. You'll ask the right questions and listen between the lines.
- Guide clients and families through our services, answer questions with clarity and kindness, and turn uncertainty into comfort and confidence.
- Schedule and conduct home visits for new and returning clients.
- Conduct in-home supervisory visits every 30 and 60 days.
- Update care plans and make necessary revisions as needed.
- Conduct home safety assessments to help maintain a safe environment for clients.
- Supervise, teach, and orient caregivers in accordance with agency guidelines and regulatory standards.
- Participate in the supervision of caregivers and recognize outstanding performance.
- Identify caregiver performance issues and address them appropriately.
- Co-manage the caregiver training program.
- Participate in quality assurance evaluations of the services provided to clients.
- Perform related duties and responsibilities as assigned by the management team.
- Be willing to assist with the two-step PPD testing program as needed.
Requirements
- Degree in Social Work, Nursing or a related field from an accredited school; a minimum of five years of experience is preferred.
- Dementia care experience preferred.
- Sales experience preferred.
- Goal-oriented team player.
- Excellent communication and interpersonal skills.
- Strong problem-solving skills with the ability to prioritize workload effectively.
- Must maintain an annual TB test.
- Able to multitask, prioritize responsibilities, and perform effectively in a fast-paced environment.
- Strong organizational skills, attention to detail, and the ability to delegate appropriately.
- Self-motivated team player who can work independently while following agency policies and procedures.
- Able to identify and analyze complex personnel and operational issues, recommend solutions, and implement appropriate actions.
