Dementia Prog Act Coord

About the role

The Dementia Program Activities Coordinator works with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias. The Coordinator models activities to family caregivers and facility staff. The Coordinator also performs bathing consultations, where they model strategies to maximize the comfort of both the person with dementia and their caregiver.

Responsibilities

  • Speaks with family members/caregiver, facility staff and Haven team to identify areas of interest and engagement opportunities for patient, barriers to bathing and/or other ADLs, and demonstrates advanced knowledge of providing engagement and care to patients with dementia, as well as available community resources for patients and caregivers.
  • Demonstrates proficiency in active listening skills and exhibits a non-judgmental, empathetic professional persona.
  • Schedules on-site visits with patient and caregiver/facility staff, creates an assortment of individualized activities, and visits patients in private homes and facilities to model engagement activities.
  • Adjusts actions based upon the situation and the response of the patient and family/caregiver, creates a customized activity guide, and completes documentation of visit in EMR.
  • Adds customized activity and ADL strategy guides to EMR and shares during IDT and/or individually with care team members.
  • Bi-monthly follow-ups with the Haven team to assess implementation of engagement and bathing/ADL strategies and offer additional suggestions.

Qualifications

  • High School diploma or GED equivalent, preferred.
  • One (1) to two (2) years CNA experience preferred.
  • Experience working with interdisciplinary team preferred.
  • Current Certified Nursing Assistant Certificate required.
  • Driver's license and insurance required.

Special Requirements

  • Travel Requirements: 25-75%
