About the role
The Dementia Care Navigator at Trustbridge Hospice, a member of Empath Health, supports individuals living with dementia and their caregivers. The role involves conducting assessments, providing navigation services, and supporting families through the dementia journey.
Responsibilities
- Conduct psychosocial assessments for people living with dementia and their caregivers as part of the Dementia Education Program team.
- Provide Care Navigation services, including initial GUIDE screenings/assessments and ongoing monitoring and support for patients enrolled in the GUIDE program.
- Support individuals with dementia and their caregivers/families across program needs, offering guidance, resources, and intervention.
Requirements
- Degree in Social Work.
- Valid Florida driver’s license, reliable automobile, and current auto insurance.
- Minimum of (1) one year experience as a caseworker with an emphasis on counseling, crisis management, community agency referral and grief support, preferred.
- Experience in developing collaborative relationships with referral sources and first responders.
- Ability to write routine reports and correspondence.
- Expertise and knowledge regarding older adults, dementia, dementia care and support, family and informal caregiver needs and challenging behaviors, preferred.
- Skills and experience in facilitating small group learning and discussion programs; including support groups.
- Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.

Schedule
Full-time position works Monday through Friday, 8:30 a.m. – 5:00 p.m.
