About the role
The patient care coordinator at Ochsner Health provides medical guidance, supports patients and their families, and helps them navigate the diagnostic evaluation and treatment process. They also educate patients about their conditions, review medications, discuss advanced care planning, and assist with home safety.
Responsibilities
- Recruits and screens care program participants for eligibility
- Coordinates and oversees patient care activities
- Maintains required documentation in compliance with protocols and regulations
- Escalates caseload needs to appropriate clinical team members
- Attends team meetings, case reviews, and performance reviews
- Performs other duties as assigned
Requirements
- High school diploma or equivalent
- One year of experience with direct patient contact
Qualifications
- Effective verbal and written communication skills
- Strong organizational, time management, and prioritization skills
- Ability to work independently and collaboratively
- Proficiency using computers and software
- Ability to build rapport and support patients and families
Skills
- Ability to work in a fast-paced environment
- Knowledge of Good Clinical Practice Guidelines
- Understanding of relevant privacy laws and institutional policies
