Responsibilities
- Utilize your expertise in case management to assess, plan, implement, monitor, and evaluate patient care
- Collaborate with healthcare professionals to develop and implement individualized care plans
- Conduct patient assessments and provide ongoing monitoring of their condition
- Cookordinate and facilitate communication between people supported, staff, families, and healthcare providers
- Utilize your knowledge to interpret diagnostic evaluations and develop appropriate treatment plans
- Provide education and support to people we support, staff and their families regarding their healthcare needs
- Assure that nursing case delegation and management provided by nursing is within standards of COMAR 10.27.11, 10.22, and is evidence-based best practice
- Utilize your skills in utilization management to ensure appropriate utilization of resources
- Collaborate with primary care physicians and other healthcare providers to ensure continuity of care
- Apply your knowledge of managed care principles to ensure cost-effective care delivery
- Utilize your expertise in case management to coordinate services across various healthcare settings
Requirements
- Valid RN license in Maryland
- A minimum of two years of experience in a DDA or related setting is preferred
- Two Years Working with developmental disabilities through the state of Maryland
- Strong knowledge of patient assessment techniques and vital signs monitoring
- Proficient in utilizing electronic medical records systems
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
- Strong organizational and time management skills
- Ability to prioritize tasks and manage multiple cases simultaneously
- Knowledge of primary care principles and practices
- Familiarity with managed care concepts and utilization management
