Position Details

Location: Apple Valley, MN (55124). Hours: Days Monday, Tuesday & Wednesdays 12‑hour shifts. Client specifics: 3‑year old with a trach, vent, and a g‑tube. Additionally: 12‑hour overnight shifts – overnights every Sunday, Wednesday & Saturday night, and every other Thursday & Friday night. Client specifics: 1‑year old with a trach, vent, and a g‑tube.

Responsibilities

• Provide skilled nursing care in home and school settings
• Assess and monitor your patient's condition and communicate changes with the care team
• Administer medications, treatments, and prescribed therapies
• Provide trach, ventilator, airway, and G‑tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment

Shift Differentials

Weekday Nights (Mon–Fri): +$1/hour
Weekend Nights (Sat–Sun): +$2/hour
Weekend Days (Sat–Sun): +$1/hour

Pay Range

LPN: $25.00 – $31.00/hour
RN: $35.00 – $41.00/hour
Starting pay is based on license, relevant experience, and qualifications.
