Day Habilitation Community Integration Professional I

This role operates in a Day Habilitation (DAY HAB) Setting, defined as services “that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment.” (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. Community Integration Professionals support the mission of the organization – empowering people to reach individual achievement across the spectrum of life.

About the Role

As an integral part of the overall supported employment team, you will collaborate with other Community Integration Professionals, Direct Support Professionals, Programmers, Supervisors, individuals-served, and their families to ensure the best possible care for those you serve. You will creatively strategize ways for an individual to find employment that suits their skills, abilities, and goals. Your innovative and impactful solutions and strategies will make the difference for those you serve.

Responsibilities

• Person-Centered Community Engagement: Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
• Goal-Based Skill Development: Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction rather than doing tasks for them.
• Behavioral Support Implementation: Follow individualized behavioral care plans by applying approved strategies, encouraging participation, motivating progress, and supporting positive behavior in alignment with each person’s goals.
• Documentation & Progress Tracking: Maintain accurate, consistent, and timely documentation of individual progress, activities, and outcomes to ensure continuity of care and accountability.
• Safety & Advocacy: Provide a safe, supportive environment while respecting individual choice and autonomy, advocating for independence, dignity, and person-directed decision-making in all aspects of service delivery.

• Valid driver’s license and reliable transportation for transporting individuals to employment opportunities.
• Automobile insurance covering $100,000/$300,000/$50,000 liability.
• High school diploma or equivalent.
• Authorization to work in the U.S. without sponsorship.

Physical Demands

• May require lifting up to 35 pounds; assistance or equipment required for heavier lifting.
• May require prolonged sitting and working at a computer for up to eight hours.
• May require frequent bending, stooping, and stretching.
• May require prolonged standing for up to eight hours.
• Must wear hearing and eye protection in required areas.
• Must have the ability to see objects/persons at a distance and close work such as typed or handwritten material.
• Must have the ability to hear conversations in both quiet and noisy environments, determine sound direction, and distinguish among bells, buzzers, beeps, horns, etc.
• Must have the ability to communicate through speech.
• Must be able to drive and transport individuals if needed.
• Must remain awake during working hours, if applicable.

Pay

$15/hour, with potential for higher pay depending on experience and education.

Schedule

This is a full-time position. Scheduling is based on program needs, as we are a 24/7 service provider.
