Propose a service plan within the stated timeline and thereafter continually monitor a plan which will delineate individual service needs and a plan to access the services. For IDD clients, efforts will be made to prevent or delay institutional placement and to enhance inclusion in the community. Provide transportation for clients as needed and directed by supervisor. Serve as a link between individuals, their families, Life Help services and the community. Provide individuals and their families with information and education concerning diagnosis, medication and A&D issues. Conduct home visits and assist the consumer in availing themselves of needed community services. Work with the family or other responsible parties of each consumer enrolled. Submit weekly billing slips to the CSS Quality Assurance for SED clients. Submit all monthly reports required by the Department of Mental Health and Life Help. Provide medication monitoring as needed and consult with appropriate medical staff. Encourage individuals to attend all needed and appropriate appointments. Qualifications Bachelor's Degree in a related mental health field. Valid driver's license and reliable personal vehicle. Case Management Certification from Mississippi Department of Mental Health.
