Role and Responsibilities
- Obtain and maintain certification to perform CSS, including completing annual training and submitting applications independently.
- Meet productivity goals through planning and support from management.
- Provide case management, ongoing client assessments, and develop treatment plans.
- Cookordinate services with other agencies and offer supportive counseling to clients and families.
- Assist clients with benefits, housing, medication management, and access to community services.
- Conduct home visits based on client needs and advocate for clients.
- Provide family counseling and education about mental illness.
- Maintain a caseload with proper documentation, attend meetings, and submit reports.
- Handle tasks related to billing, authorization, and compliance with payors.
- Participate in supervisory meetings, training, grant activities, and networking.
- Perform additional duties as assigned.
Qualifications
- Bachelor’s degree in social work, counseling, psychology, or related field.
- Ability to relate effectively to severely mentally ill individuals.
- Valid driver’s license, automobile insurance, safe driving record, and transportation.
- Maintain a Communicare insurable driving record.
