About the role

The Clinical Support Specialist works under the supervision of the Program Director/Mallard Site Manager to provide referral and patient flow support to NHFHS Care Teams.

Responsibilities

  • Demonstrates a working knowledge of the EHR, clerical, and referral processes.
  • Receives and processes patient-related information from multiple sources, ensuring timely and accurate delivery to the appropriate departments or individuals.
  • Helps support the achievement of provider productivity goals.
  • Establishes and maintains effective working relationships with all members of the health care team and demonstrates appropriate communication skills.
  • Processes provider referral orders in a timely fashion and forwards necessary EHR information as required for each individual order.
  • Schedules In-House referrals as needed and follows established processes and guidelines.
  • Schedules External referrals for various procedures, including radiological procedures and consultations with specialists as ordered by medical providers, and directs patients accordingly.
  • Obtains necessary referral pre-authorizations.
  • Forwards records for referral orders via EHR Fax.
  • Communicates all "stat" reports/issues immediately to appropriate team members (MD, FNP, RN/LPN, MA, PM, or CN).
  • Reports immediately to the referring Provider if unable to complete a referral.
  • Understands and applies Patient Centered Medical Home concepts.
  • Tracks outcomes for referrals, hospitalizations, pap smears, vouchers, and other required information supporting Patient Centered Medical Home guidelines, working with Task Management and clinical staff as needed.
  • Communicates clerical supply needs to the immediate supervisor in a timely manner.
  • Secures necessary referral pre-authorizations in accordance with payer guidelines and organizational policies.
  • Contributes constructive ideas for a more efficient referral process.
  • Attends staff development programs, monthly staff meetings, and developmental programs.
  • Maintains confidentiality of patient information at all times.

Requirements

  • Education:
    • Required: High school diploma or GED equivalent. CNA, MA, PCT, or minimum of 1 year of related work experience.
    • Preferred: 1-2 years higher education in a related field or social work fields.
  • Experience:
    • Required: Electronic Health Records Experience.
    • Preferred: One year experience within a community healthcare setting.
