About the role
The Critical Time Intervention (CTI) Coach is the direct contact for clients under the CTI model. This role involves a nine-month intervention focusing on connecting clients to long-term support services.
Responsibilities
- Rapidly engage referred individuals and facilitate enrollment
- Assess barriers and develop comprehensive plans to connect to appropriate supports
- Strengthen supports and connect to community providers
- Ensure long-term stability for the client and maintain all long-term services
Requirements
- Bachelor’s Degree
- Experience in behavioral health and/or social determinants of health preferred
- Experience with social determinants of health-related programs, including housing, employment, food insecurity, and entitlements preferred
- Understanding of NH systems of care, with experience working between systems preferred
- Effective communication skills, both oral and written
- Ability to utilize technology provided to perform job requirements
- Valid driver’s license, proof of adequate automobile liability insurance, and reliable transportation required
Qualifications
- Passport to vaccine against or willingness to get vaccinated against COVID-19 by the date of hire
- Pass background check including criminal and driving records

Pay
$X per hour or $Y per month, depending on qualifications and experience
