Job Summary

Becomes part of a supportive and collaborative healthcare community dedicated to making a positive impact on patients' lives.

Responsibilities

  • Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in assessments.
  • Reviews and updates treatment plans every 30 days.
  • Performs a mini mental status exam and psychosocial assessment on all new patients.
  • Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines.
  • Performs other duties as assigned.

Qualifications

  • Education: Master's degree in Social Work or related field.
  • Experience: 2 years of related experience.
  • Licensure/Certification: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy.
  • CPR

Salary

$36.95 - $57.27 per hour. Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience.
