CPW Liaison/QA Specialist

About the role

The Program Specialist III serves as a liaison and trainer for Case Management for Children and Pregnant Women (CPW) program providers, regional case managers, and potential providers. They also perform quality assurance reviews and coordinate outreach efforts.

Responsibilities

• Liaison to the Children and Pregnant Women (CPW) Program
• Provides complex and specialized social work consultation and coordination of services to families of children in the Personal Care Services (PCS) Program and Community First Choice (CFC) programs.
• Maintains documentation of work activities with record documentation, monthly reports, travel reports, case summaries and correspondence.
• Conducts CPW Pre-planning Sessions with potential providers and follows them through the application process.
• Works in collaboration with other DSHS SHSS Liaisons from other regions as needed.
• Collaborates with HHSC Case Management Division on special assignments.
• Conducts outreach and actively collaborates with community partners, by participating in various community organizations such as the CRCG and local coalitions.
• Conducts presentations and attends community events to promote, educate and inform the general public about CPW and other SHSS program services.
• Communicates effectively and professionally with clients, colleagues, supervisors, as well as the general public.
• Attends trainings and conferences relevant to social work licensure requirements and continued development of professional social work knowledge and skills.
• Assists in conducting new SHSS staff training upon hiring of new employees.
• Represents PHR 7 on teleconferences relating to Case Management Program.

Requirements

• Bachelor's degree (minimum) in social work or a related field, along with a current unrestricted license to practice in Texas as a Licensed Social Worker.
• A minimum of 3 years working in a governmental, public health, or human services setting performing a social worker role (case management, healthcare navigation, needs assessments, and/or case reviews).
• Experience with Personal Care Services, Community First Choice, Children with Specialized Health Care Needs, and Texas Health Steps is preferred.

Qualifications

• Knowledge of case management.
• Knowledge of children’s programs, such as CPW/Case Management, Medicaid, CSHCN, CHIP, PCS and MTP.
• Knowledge of technical assistance and quality assurance practices.
• Knowledge of local, state and federal laws related to the program area.
• Knowledge of research techniques, training and marketing techniques and program management processes and techniques.
• Skills in using computer software programs in word processing, spreadsheets, power point, electronic mail and internet.
• Skills in program planning.
• Skills providing presentations and trainings to individuals and groups.
• Skills in conflict resolution, mediation, and problem solving.
• Skills in public speaking.
• Ability to plan, prioritize, implement and analyze outcomes of activities.
