About the Role
The Comprehensive Prenatal Services Program (CPSP) Coordinator assists in the case management of prenatal patients under the direct supervision of the CPSP Supervisor. This role involves family planning client orientation, comprehensive prenatal services assessments (initial, trimester, and postpartum), interventions, referrals, and follow-up services across Obstetrics, Nutrition, Health Education, Individual Case Coordination, and linkages with programs such as WIC, family planning, and pediatric care.
Responsibilities
- Implement and integrate the CPSP Program, including nutrition, psychosocial, and health education assessments, interventions, and perinatal education with basic obstetrical care.
- Ensure the integrity and quality of health services delivered in Comprehensive Prenatal Services, complying with Federal, State, and L.A. County Standards, as well as site-specific policies and protocols.
- Represent the clinic in matters related to the delivery of these services, collaborating closely with the Clinic Administrator, consultants, and medical providers to develop and implement CPSP program goals and objectives.
- Provide assessment, documentation, and program eligibility for the CPSP program.
- Establish and maintain open communication with the Los Angeles County Department of Public Health - Comprehensive Prenatal Services Program.
- Ensure CPSP program services are delivered effectively.
- Direct the maintenance of program logs and statistics for internal and external review and reporting.
- Conduct follow-ups and documentation of postpartum delivery and lactation education.
- Assure VCCHC representation at designated meetings.
- Participate in Continuous Quality Improvement plans for the site and program.
- Perform other tasks as assigned.
Schedule
Full-Time, Monday - Friday.
