Coordinator, Prenatal Support Service

Responsibilities

  • Affirms prenatal program services to FHCSD staff and outside agencies.
  • Builds positive relationships, inspires others to perform at a higher level, and prepares career development plans.
  • Counsels and coaches staff and administers appropriate disciplinary action in a timely manner.
  • Gathers program data, conducts analysis, and provides reporting, including productivity reports.
  • Maintains a quality monitoring program, conducts quality assurance checks, and ensures standardization, including EHR reviews/audits, patient satisfaction surveys, staff training and development, and other measures.
  • Ensures program compliance with program requirements.
  • Oversees daily prenatal support services, including health education and social services, at assigned sites.
  • Oversees and monitors scheduling to ensure maximum utilization and productivity, developing and monitoring patient flow systems, coordinating hospital tours, ordering supplies, and ensuring health education and psychosocial coverage at sites/hospitals.
  • Participates in continuous improvement activities and ensures appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
  • Provides direct patient services, including conducting prenatal health education, trimester assessments, post-partum assessments, and hospital tours.
  • Ensures recommendations made on care plans for support services interventions are carried out and implemented.
  • Provides/ensures staff receive all new hire and annual trainings on time.

Requirements

  • 1 year experience in maternal child health, perinatal care, and staff supervision required.
  • 5 years of progressively responsible positions in the healthcare industry or out-patient medical clinics required.
  • Bachelor's degree in Health Science or Social Science field required.
  • Certified Childbirth Educator preferred.
  • Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Traveling between sites and other locations is required as an essential function of the job.
  • Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended.
  • An acceptable driving record is also required.
  • California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency.
  • Mileage and other reimbursement governed by policy.
  • Ability to work effectively with patients from diverse social, cultural and economic groups.
  • Ability to work well in both a team-based environment and independently.
  • Bilingual in Spanish/English required.
  • Demonstrated ability to be culturally sensitive and respect diversity.
  • Intermediate computer literacy ability to comply with department needs and expectations, including MS Office.
  • Intermediate interpersonal and customer service skills.
  • Intermediate written and verbal communication skills.
  • Knowledge of perinatal programs, care management, and procedures.
  • Knowledge of supervisory practices and techniques, including performance management and employee development.
  • Strong analytical and problem solving skills.
  • Successful track record of leading and driving positive change through others.
