Job Description
Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components.
- Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
- Safeguards the provision of high quality, individualized, comprehensive, cost-effective patient care across the continuum.
- Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
- Collaborates with interdisciplinary medical care, liaises with patient, family, medical staff, and referring physicians.
- Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
- Develops clinical protocols, policies, and procedures including community education and public relations.
Education Requirement
Bachelor's Degree, required. Masters Degree in Nursing, preferred.
Licensure Requirement
Registered nurse in the State of Ohio, required.
Skills
- Effective customer service skills.
- Good multiplatform communication skills (oral, written, presentation).
- Ability to assist in conflict resolution.
- Problem-solving and interpersonal skills required for interaction with contacts required.
Experience
Three years of registered nurse experience, required.
