The Heart and Vascular Nurse Navigator Coordinator provides a link for patients to physicians and to services at the health system. An experienced cardiovascular clinician, the navigator is available to understand patients' individual needs and to supply education, information, and scheduling resources. The role expedites prompt patient treatment and provides exceptional support and answers ongoing questions for patients and families throughout the course of their care.

Responsibilities

  • Provides oversight to services provided by registered nurse navigators and provides some patient nursing care for patients and/or patient populations.
  • Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
  • Assists patients and their families as a resource for appointments and necessary services, including outpatients and inpatients following Cardiovascular surgery and PCI.
  • Provides prompt follow-up to patients placed in Accelerated Low Risk Chest Pain Protocol from the Emergency Department, ensuring cardiology follow-up.
  • Supports the interdisciplinary team for valvular heart disease patients during Valve Center Clinics, ensuring quality and comprehensive coordinated services and follow-up.
  • Actively maintains the Valve Center Database to ensure timely notification of abnormal results to physicians and appropriate follow-up care for patients.
  • Supports planning of cardiovascular patient education and outreach programs, including awareness programs and community lectures; assists in follow-up activities for cardiovascular screening events.
  • Coordinates and schedules tests and appointments for patients by communicating with physicians’ offices to ensure they are performed in a timely manner.
  • Maintains contact with cardiac patients as needed to answer ongoing questions, reduce stress, and provide available services.
  • Identifies and discusses cardiac risk factors with patients who have participated in the online Heart Tracks cardiac risk assessment tool.
  • Communicates patient needs, with attention to details, to ensure a safe level of care and appropriate resources after discharge.
  • Performs as a team player, open to change and different approaches to work.
  • Recommends new approaches to solving operational problems, issues, or concerns.
  • Identifies opportunities to improve patient care for cardiovascular patients by changing and developing new processes.
  • Acts as a facilitator for the monthly cardiac support group and the Mended Hearts volunteer program.

Requirements

  • Registered Nurse - PA (Required)
  • Cardiology Related Certification (Preferred)
  • Bachelor of Arts or Science (Required)
  • 5+ years of Cardiovascular Nursing Experience
