RN Cardiology Coordinator

About the role

The RN Coordinator is a member of the multidisciplinary team working in collaboration with the physician. The RN utilizes the care process consistent with their license or certification, including initiation of interventions within individual scope of practice. The RN Coordinator oversees coordination of the multidisciplinary plan of care and ensures its consistency with the medical treatment plan.

The patient care process includes:

• Assessment: Identification, gathering, and organization of subjective and objective data pertaining to the patient, family, or significant other.
• Planning: Identification of actual or potential problems and desired outcomes (goals) resulting in a documented plan of care.
• Intervention: Implementation of specific interventions consistent with the plan of care and medical treatment plan.
• Evaluation: Determining the effectiveness of interventions and documenting required actions, including reassessment and revisions.

Responsibilities

• Assist Cardiologists with medication and call volume triage.
• Support clients with pre-op visits, post-op visits, and wound checks.
• Screen potential clients for eligibility for the Pacemaker Clinic.
• Complete health assessments on clients for the Pacemaker Clinic.
• Facilitate appointments, services, follow-up testing, and coordination of care.
• Provide wellness activities to program participants.
• Participate in treatment planning and act as a liaison to the community.
• Assist with required data collection on clients enrolled in the Pacemaker Clinic.
• Participate in weekly staff meetings.
• Assist with care transitioning of program participants.
• Provide and assist with linkages to community services.
• Assume responsibility for continued professional and personal growth.
• Support organizational goals through quality customer service, performance improvement efforts, and teamwork.

Requirements

• Graduate of an accredited nursing program; Bachelor’s Degree in Nursing preferred.
• Current licensure as a Registered Nurse in the State of New Jersey.
• Valid New Jersey Driver’s license preferred.
• BLS for Healthcare Providers (BLSHCP) required.
• ACLS required (not required for Urgent Care Settings).
• 3–5 years of clinical experience; demonstrated leadership ability and strong organizational skills preferred.
• Proficiency in Clinical Applications preferred at hire; training provided with skill required within 30–60 days.

Work Environment

• Potential exposure to hazards of the hospital environment, bloodborne pathogens, infectious disease, hazardous substances, and potential injury.
• Physical demands include considerable reaching, stooping, kneeling, crouching, extensive standing, sitting, and walking for the majority of the workday.
• Requires lifting approximately 25–50 pounds with assistance.
• Periodic travel to company off-site locations expected.
• High mental stress demands due to handling multiple interactions with constant interruptions.

This position reports to the Practice Manager or Administrator-Ambulatory Services.
