Essential Functions/Responsibilities
- Affords instruction and supervision to people participating in the Community Services Programs, including Community Habilitation and In Home Respite.
- Collaborates with individuals in their own homes and in the community to enhance their independence in activities of daily living.
- May conduct assessments of new program participants.
- Provides training and assists with orientating newly hired staff.
- Assists the Senior Community Supervisor with monthly summaries, reviewing billing, writing Staff Action Plans, and attending Life Plan meetings.
- May assist the Senior Community Supervisor with covering cancellations/staff PTO as needed.
Activities of Daily Living
- Gaining and maintaining independence in areas of daily living skills.
- Providing a break to the Caregiver (In Home Respite only).
- Accessing and/or being involved in the community.
- Learning social skills while at home and/or in the community.
- Other duties as outlined in the Staff Action Plan and as assigned.
Supervisory Responsibilities
This position has no direct supervision responsibilities.
Minimum Qualifications
- Must be 18 years of age.
- One to two years of experience preferred.
- High School Diploma or GED.
- Valid Driver’s License meeting agency policy.
- Ability to transport individuals in a personal vehicle meeting agency requirements.
- Physical ability to perform tasks such as standing, walking, sitting, bending, squatting, kneeling, and climbing.
