COORDINATOR, COMMUNITY BASED SERVICES

Essential Functions/Responsibilities

• Affords instruction and supervision to people participating in the Community Services Programs, including Community Habilitation and In Home Respite.
• Collaborates with individuals in their own homes and in the community to enhance their independence in activities of daily living.
• May conduct assessments of new program participants.
• Provides training and assists with orientating newly hired staff.
• Assists the Senior Community Supervisor with monthly summaries, reviewing billing, writing Staff Action Plans, and attending Life Plan meetings.
• May assist the Senior Community Supervisor with covering cancellations/staff PTO as needed.

Activities of Daily Living

• Gaining and maintaining independence in areas of daily living skills.
• Providing a break to the Caregiver (In Home Respite only).
• Accessing and/or being involved in the community.
• Learning social skills while at home and/or in the community.
• Other duties as outlined in the Staff Action Plan and as assigned.

Supervisory Responsibilities

This position has no direct supervision responsibilities.

Minimum Qualifications

• Must be 18 years of age.
• One to two years of experience preferred.
• High School Diploma or GED.
• Valid Driver’s License meeting agency policy.
• Ability to transport individuals in a personal vehicle meeting agency requirements.
• Physical ability to perform tasks such as standing, walking, sitting, bending, squatting, kneeling, and climbing.
