About the role

The Advance Care Planning (ACP) Program Coordinator facilitates improvement of end-of-life health care through practice, education, evaluation, research and consultation specifically related to advance care planning. The Program Coordinator is responsible for maintaining and meeting the ACP referrals utilizing the Respecting Choices model to facilitate advance care planning conversations and assist in completing Advance Directives for Healthcare (ADHC).

Responsibilities

  • Identifies and engages key stakeholders in the goals of the advance care planning (ACP) initiative.
  • Affords support in the development of an organizational system that supports ACP goals.
  • Maintains ACP referral system.
  • Facilitates process improvement ensuring education, workflow, documentation, and measurement are in place.
  • Collaborates on design of educational training and education for staff and community.
  • Affords assistance in project management of ACP implementation and growth.
  • Affords expertise in implementing and maintaining system-wide, comprehensive ACP programs.
  • Improves patient, family and population outcomes based on research, evidence-based practice, innovative healthcare delivery systems and professional collaboration with others.
  • Performs chart reviews as necessary for utilization review of goal concordant care associated with ACP.

Requirements

  • Licensure or other certifications: Licensed to practice as an RN or LSWEducational Requirements: Bachelors Degree.
  • Minimum Experience: Three (3) years of clinical experience as an RN or five (5) years clinical experience as an LSW, LCSW, LMSW or MSW
