Consumer Services Coordinator (Case Management) for Riverside School Age East
Under the direct supervision of the Program Manager, the Consumer Services Coordinator (CSC) coordinates services and supports for persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. This position serves consumers in the Riverside East area, including Banning, Beaumont, Blythe, Cabazon, Calimesa, Cathedral City, Coachella, Desert Center, Desert Hot Springs, Idyllwild, Indian Wells, Indio, La Quinta, Mecca, North Palm Springs, Palm Desert, Palm Springs, Rancho Mirage, San Jacinto, Thermal, Thousand Palms, and Whitewater. Mileage reimbursement for business travel is provided monthly.
Hourly Range: $26.3130 - $40.8201
Responsibilities
- Provide case coordination services for consumers, utilizing Person-Centered Planning strategies to create and execute Individual Program Plans (IPP), generate progress reports, and conduct annual reviews.
- Evaluate consumer progress and update IPPs as needed, including creating and implementing IPP Addendums.
- Provide advocacy services to consumers and their families, attending relevant meetings (e.g., IEP, ITP, IHC) and preparing necessary paperwork.
- Utilize generic resources effectively on behalf of consumers and their families.
- Complete individual or family crisis intervention and document activities appropriately.
- File Special Incident Reports promptly and inform representatives of other agencies as needed.
- Coordinate consumer assessments, including medical, psychological, developmental, educational, vocational, OT/PT, and others.
- Complete placement and liaison activities as required.
- Ensure Medicaid Waiver standards are maintained and documentation is completed on time.
- Arrange for consumer placements in residential facilities, day programs, or other training programs as clinically indicated and approved by the Interdisciplinary Team.
- Complete facility audits (ICRC 513 form) and refer identified issues to Quality Assurance or Resource Development and Transportation Program.
- Keep the manager informed of caseload status, community activities, and unusual or difficult case situations.
- Participate in case conferences and interagency meetings as needed or assigned.
- Provide case coordination coverage for consumers whose CSC is absent or unassigned, as directed by the manager.
- Act as a mentor for new employees, as directed by the manager.
- Complete IRC’s orientation and new staff training sessions.
- Visit Level 2, 3, and 4 residential facilities and day programs as assigned.
- Participate in at least one annual Quality Assurance evaluation/audit of a community care facility, health care facility, or day program.
- Complete administrative requirements daily, including Purchase of Service forms, updating consumer files, and entering Target Case Management (Title 19) notes to secure at least 400 units per month.
- Ensure completion of at least 95% of required case-related paperwork within designated time frames.
- Facilitate the purchase of services identified in the IPP.
- Prepare for compliance reviews, eligibility reviews, and other administrative case reviews.
- Maintain and expand relevant knowledge by attending mandatory and authorized training sessions to earn required Continuing Education Units (CEUs).
- Work as a team member, sharing expertise and contributing to team harmony.
- Adapt to change, handle interruptions, and manage new priorities and assignments in a hybrid work environment (office, remote, and field).
- Organize travel efficiently to visit consumers, families, and vendors.
- Utilize bilingual skills in all aspects of the job, if applicable.
- Use office equipment appropriately, maintain a neat work area, and comply with safety rules and IRC’s Injury and Illness Prevention Plan.
- Utilize IRC’s IT systems and equipment securely, following protocols and procedures.
- Uphold consumers’ rights and dignity in service provision.
- Comply with IRC’s personnel policies and procedures.
- Perform additional duties as assigned.
Requirements
- Bachelor’s degree from an accredited college or university and one year of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
- OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
- Bilingual skills preferred but not required.
- Ability to establish and maintain effective working relationships.
- Good verbal and written communication skills.
- Full use of an automobile, valid California driver’s license, and liability insurance (or ability to provide independent transportation).

Case Management Trainee Option
For candidates who meet the education requirement but lack the experience, a Case Management Trainee (CMT) position is available. This is a year-long trainee role requiring a Bachelor’s degree from an accredited college or university.
