About the role
The Consumer Services Coordinator (CSC) provides case coordination services for individuals with developmental disabilities and their families. The position involves driving to various areas such as Canyon Lake, Hemet, Homeland, Lake Elsinore, Menifee, Nuevo, and Quail Valley to deliver services.
Responsibilities
- Coordinate services and supports for individuals with developmental disabilities and their families.
- Create and execute Individual Program Plans (IPPs) and conduct annual reviews.
- Evaluate consumer progress and plan as identified in the IPP on an ongoing basis.
- Provide advocacy services and attend relevant meetings.
- Complete individual or family crisis intervention and document activities.
- Complete Special Incident Reports and inform representatives of other agencies and programs as appropriate.
- Coordinate consumer assessments and complete placement and liaison activities.
- Arrange for the placement of consumers in residential facilities, day programs, and other training programs.
- Ensure Medicaid Waiver standards are maintained and documentation is completed on a timely basis.
- Participate in case conferences and interagency meetings.
- Provide case coordination coverage and mentor new employees.
- Facilitate the purchase of services identified in the IPP.
- Attend mandatory and other authorized trainings to earn CEU’s.
- Maintain and update consumer file information and documentation.
- Ensure consumers' rights and dignity are maintained.
- Follow IRC's personnel policies and procedures.
Requirements
- Bachelor’s degree from an accredited college or university and one year of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
- Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
- Bilingual preferred but not required.
- Ability to establish and maintain effective working relationships with others.
- Ability to work cooperatively and effectively with others.
- Maintain good attendance and punctuality.
- Full use of an automobile, possession of a valid California driver’s license, and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation.
- Must have and maintain a safe driving record.
Qualifications
- Experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
- Knowledge of Person-Centered Planning strategies.
- Ability to establish and maintain effective working relationships with others.
- Ability to work cooperatively and effectively with others.
- Maintain good attendance and punctuality.
- Full use of an automobile, possession of a valid California driver’s license, and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation.

Hourly range: $26.3130 - $40.8201
Schedule
Hybrid work environment consisting of working in the office, remotely at home, and in the field.
